Luke provides case management services for military healthcare clinics and medical centers. Our Case Managers provide individual patient and family level case management services, as well as care coordination for both short-term and long-term care. Their patients typically have needs that are complex and expensive. Our Case Managers work closely with the medical treatment facility staff to identify available resources to achieve optimal levels of wellness and maintain function. They are compassionate, engaging professionals who work hard at achieving optimal care, communication, education and support systems for their patients by enhancing a patient’s productivity and quality of life in a cost-effective manner by facilitating timely and appropriate health services.

Luke’s Case Management personnel are credentialed by the American Accreditation Healthcare Commission/Utilization Review Accreditation Commission (URAC) Case Management Standards, the Case Management Society of America (CMSA) and the Joint Commission (JC).
